Your meibomian glands are tiny oil glands along your eyelid margins. They produce the oil layer of your tear film. When these glands get clogged with debris, bacteria, makeup residue, or hardened oils, the tear film becomes unstable.

Sound familiar? As a dry eye specialist, I always explain to my patients that if we don’t treat the eyelids, we’re only treating half the problem.
For years diluted baby shampoo was commonly recommended for lid hygiene. But dry eye care has evolved. Today, dedicated foaming eyelid cleansers are more aligned with current, evidence-based dry eye management:
1. Better Protection of the Tear Film
Baby shampoo is a detergent. Even when diluted it can disrupt the lipid layer of the tear film, the exact layer we are trying to protect. A poor lipid layer increases tear evaporation and worsen symptoms in patients with evaporative dry eye.
2. They’re Gentler on the Ocular Surface
Baby shampoo contains surfactants, preservatives, and sometimes fragrance that aren’t safe for long-term use on the delicate eyelid area. With chronic use, these ingredients can cause low-grade irritation and ongoing inflammation — the exact opposite of what we’re trying to achieve in dry eye care.
3. They Address Biofilm and Demodex
Bacterial biofilm and microscopic Demodex mites commonly contribute to blepharitis and MGD. Many foaming cleansers contain ingredients like tea tree derivatives or hypochlorous acid to help manage these contributors, something baby shampoo was never designed to do.

The ocular surface has a tightly regulated pH, baby shampoo is typically more alkaline than the tear film. When eyelid cleansers are formulated closer to physiologic pH, it helps:

Dry eye is now recognized as a chronic inflammatory disease. Baby shampoo is a general cleanser approach - not inflammation-aware therapy.
Foaming eyelid cleansers are complement other therapies such as lipid-based artificial tears, prescription medications, IPL, RF and in-office gland treatments.
One of the most empowering parts of dry eye management is giving my patients tools they can use daily at home. Here’s a simple routine:
1. Wash your hands thoroughly.
2. Dispense foam onto clean fingertips.
3. With eyes closed, gently massage along the lash line and eyelid margins. Let sit for 1 minute.
4. Rinse with warm water and pat dry.
I typically recommend using it once or twice daily, depending on the severity of symptoms. For many of my patients, this becomes the foundation of their long-term maintenance plan.
